This position establishes professional relationships with physicians, discharge planners, social workers, facilities and health care decision makers by providing on-going education regarding hospice services and care. The Senior Patient Care Manager will also manage a team of regional Patient Care Representatives (PCRs).Qualifications: Must have a Bachelor's Degree in Marketing, Communications, or have 2 years progressive experience in marketing, sales, or physician services setting; Must be a self-starter, be disciplined, ambitious, and a motivated individual; Must be able to multi-task, and have good business and communication skills; Existing referral source relationships are highly desired; Must have general clerical skills and basic computer knowledge Must have the ability to utilize office equipment; Must have excellent written and oral communication skills; At least one year's experience in the hospice or health field preferred; Current automobile insurance and valid driver's license, maintain transportation to be used for work.Job Duties: Develops and strengthens new and existing referral base; Contributes to the revenue and income growth of the agency generating patient referrals from various referral sources; Develops and implements strategic marketing plan; Meets or exceeds all growth and development targets and actively establishes and maintains market awareness; Cultivates and works with business (contract) customers and prospects; Speaks to groups, organizations, businesses, the media and other entities to educate them on programs; Shares information with other staff to ensure optimal team performance; 80% of time for this position will consist of calling on new and existing accounts and 20% will consist of managing a team of 3-6 PCRs Supervises and manages a team of regional PCRs Conduct monthly field visits along with each PCR Ensure that PCRs are provided with the training and resources they need to be successful in their respective market Represent the company in a positive and professional manner by exhibiting professionalism; Use good judgment and problem-solving skills to assess the situations, consider alternatives, discuss with supervisor and choose the appropriate course of action; Participate in education and learning assignments; Development short term and long goals, setting key objectives; Attend required staff meetings; Maintain and apply knowledge of Company Policies, Ethics, and Compliance Standards; Includes the use of personnel vehicle with a standard mileage reimbursement rate; Requires the use of GPS Monitoring system at all times during working hours ; Other duties as assignedAll inquiries will be kept confidential.
