Job Description:

Job Description Job Description We embrace uniqueness. You should, too. Every day, we make a difference in the lives of our patients and team members. Are you driven by innovation and entrepreneurial spirit? Is your ambition and work ethic exceptional? Do you uplift others with your positivity and compassion? We stand apart from typical primary care providers. As we expand rapidly, we are on the lookout for exceptional individuals to join our team. Our organization, led by physicians and driven by a mission, is recognized as one of the most successful full-risk Medicare Advantage providers in the country. We aspire to be the foremost primary care provider in America, dedicated to transforming the care of the most vulnerable populations. Our mission is to honor seniors by providing affordable VIP care that enhances their health. To realize our vision and fulfill our mission, we seek outstanding primary care providers who are eager to find purpose and personal growth within our family. The Primary Care Physician (PCP) within our organization is expected to: Take Responsibility for Outcomes: The PCP is accountable for delivering strong clinical care and cost-effective outcomes for each patient in their panel, which may include up to 450 patients. They recognize the importance of building trust and fostering behavior change to positively influence patient outcomes. Act as a Health Coach: Rather than merely consulting on illnesses, the PCP helps patients establish both short-term and long-term health objectives, collaborates with them to achieve these goals, and regularly follows up to ensure progress towards improved health. Simplify for Action: The PCP prioritizes tasks effectively, making behavior change more achievable for patients and guiding their care teams towards performance goals. As a value-driven, outcomes-focused organization, we regularly assess various metrics for each PCP's patient panel to support their success and help them achieve partnership status. These metrics include patient admissions per thousand, appointment utilization (18-21 slots daily), CGCAHPS (patient experience), clinical gap closures, and medical cost measures. Each PCP will have specific targets for these metrics and will collaborate with center and market leadership, as well as care teams, to meet these goals. A positive culture is vital in our medical centers, and as leaders, PCPs are expected to promote a culture of love, accountability, and passion alongside center leadership. The PCP must demonstrate the ability to work both independently and collaboratively with other healthcare professionals. They will closely coordinate with relevant managers and medical directors to ensure adherence to guidelines and participate in risk and quality management programs, clinical meetings, and other necessary gatherings that support patient health and organizational objectives. The PCP will comply with departmental goals, performance standards, regulatory requirements, quality patient care standards, and organizational policies. KEY JOB DUTIES AND RESPONSIBILITIES: The PCP independently manages care for patients with both acute and chronic conditions, particularly in older adults. They take full responsibility for patient care and outcomes, seeking specialist consultations when necessary while remaining involved in the patient's comprehensive care. Engagement with hospitalists is expected whenever one of their patients is hospitalized, regardless of the hospitalist's affiliation. The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with complex health needs. They lead a care team that includes a medical assistant, care facilitator, and care coordinator for patients who can visit the office. For patients unable to attend the office—whether in hospitals, skilled nursing facilities, long-term care, or homebound—the PCP collaborates with the transitional care team and other resources available in the market. The PCP plays an active role in managing their center and may cover for other providers as needed. They are also expected to assist in recruiting patients and additional providers for the organization. They supervise, collaborate with, and are accountable for the actions of Advanced Practice Practitioners (APPs) within a practice or collaborative agreement. Other duties may be assigned at the discretion of management. KNOWLEDGE, SKILLS, AND ABILITIES: Competencies for Success Availability and Accessibility: PCPs should be approachable and available to their patients, fostering trust through timely responses to walk-in visits, phone calls, and messages. Service Orientation: PCPs deliver care that they would want for their own family members, ensuring a compassionate approach at every interaction. Evidence-Based Medicine: PCPs stay informed about evidence-based practices while recognizing the significant impact of lifestyle and nutrition on health outcomes. They remain updated on clinical, nutritional, and lifestyle interventions. Physician Leadership: Continuous development of leadership skills is essential for the benefit of patients, teams, centers, and the organization. Quality: Our patients deserve the highest quality of care, necessitating collaboration with care teams to achieve excellent outcomes and quality measures. Influence: PCPs must effectively and compassionately influence patients, teams, and themselves to achieve optimal outcomes. Self-Care: PCPs can best care for their patients when they prioritize their own physical, mental, social, and spiritual well-being. Physician wellness is crucial for sustainability and promoting health among physicians, staff, and patients. Proficiency in Microsoft Office Suite, including Word, Excel, PowerPoint, and Outlook, as well as other relevant software. Willingness to travel locally as needed, and occasionally regionally or nationally for training or recruitment purposes. Fluency in English, both verbal and written. Some positions may require proficiency in additional languages, which will be specified during the application process. This role requires independent judgment and decision-making. EDUCATION AND EXPERIENCE REQUIREMENTS: MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty is required. A current, active State Medical License for the state(s) of practice is mandatory. Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is preferred; board eligibility is required. Once board certified, the PCP must maintain certification through necessary MOC, CME, and/or retaking board exams as required. A current DEA number for controlled substances (schedules II-V) is required. Basic Life Support (BLS) certification from the American Heart Association or American Red Cross is required within the first 90 days of employment. SALARY RANGE: $214,700 - $306,714 EMPLOYEE BENEFITS We are dedicated to transforming healthcare for seniors and improving the overall healthcare landscape in America. As a family-owned and physician-led organization, our distinctive approach enables us to enhance the health and well-being of the populations we serve. Our rapid growth reflects our commitment to rescuing more seniors from inadequate healthcare. We are making a significant impact on the lives of those we serve and those we employ. With competitive compensation, comprehensive benefits, opportunities for career development and advancement, our employees enjoy a healthy work-life balance and numerous growth opportunities. Join our team and make a difference in people's lives every day. Current employees should apply through the designated channels. Current contingent workers should refer to the job aid for application instructions.
