Wraparound Facilitator - Position Overview
Wraparound is an approach to individualized care planning that
builds on the collective action of a committed group of family,
friends, community, professional, and cross-system supports
mobilizing resources and talents from a variety of sources to
address the needs of children/youth with complex mental health
challenges and their families.
The MYPAC Wraparound Facilitator is responsible for engaging the
family and developing a strength based relationship to identify,
create and build a supportive Child and Family Team for the purpose
of developing a single plan of care in accordance with high
fidelity and quality wraparound practice.
Engage the family to begin the process of high fidelity wraparound
and educate family, child/youth, and new team members about the
Wraparound process and components of wraparound.
Gather information for the development of the family story and
assist family with developing their family vision to present at the
first 14 day CFT meeting.
Complete the initial Crisis Management Plan with the family within
24 hours of admission to MYPAC.
Engage families in order to identify natural/formal/informal
supports for the family and child/youth and have them present at
the initial CFTM.
Responsible for maintaining team cohesiveness and providing an
environment that is conducive for all team members to have
supportive and positive communication between the team and
Facilitate a Child and Family Team meeting every 30 days at a
minimum, more frequently in the initial stages of development, and
as often as needed to provide quality services
Bachelor's degree required in Social Work or a related field.
Experience working directly with children and families and
formal/informal systems required.
Must also be able to successfully complete the Mississippi
Department of Mental Health Community Support Specialist trainings
and certification exam for certification as a Certified Community
Support Specialist (CCSS) within 90 days of hire.
Must be able to demonstrate applicable skills necessary for high
fidelity wraparound practice. Must demonstrate strong oral and
written communication skills. Computer skills are required.
Must be available to work a flexible schedule to accommodate
occasional evening and weekend hours to meet job
Must be at least 21 years of age, have a valid Mississippi driver
license, a good driving record, proof of legally appropriate auto
liability insurance, and must be willing to use personal vehicle
for organization business.
