challenges.MYPAC Therapist - Position OverviewThe MYPAC therapist
is charged with the delivery of all mental health services (e.g.,
individual therapy, family therapy, group therapy, substance abuse
treatment, etc.).The therapist completes all required clinical
assessments.The therapist is responsible for the clinical oversight
and clinical interventions related to the crisis management plan,
the individualized service plan and Wrap-Around Meetings in
coordination with the Primary Service Coordinator.The therapist is
responsible for providing targeted clinical interventions based on
the identified youth's needs through the various assessments and
plans.The therapist completes required assessments and case
documentation for each individual in a timely manner and within
program requirements.Job Responsibilities:Works in collaboration
with the MYPAC Wraparound Facilitator to address client's mental
health service needs. Conducts admission evaluations and develops
the Preliminary Action Plan within 24 hours of admission to
MYPAC.Completes programmatic assessments as assigned within
designated timeframes and uses feedback from the assessments to
plan for clinical interventions with client. Completes initial
clinical goals, objectives and interventions for the Individual
Service Plan within fourteen days of admission in coordination with
the youth, family, and Wraparound Facilitator.Co-facilitates the
initial Child and Family Team meeting with the Wraparound
Facilitator within fourteen days of admission and ongoing monthly
meetings thereafter or as needed.Completes monthly ISP Reviews,
monitors and documents progress toward treatment goals. Implements
clinical treatment interventions based on the ISP and
individualized needs of the child/family.Conducts individual
therapy sessions at least once a week, family therapy sessions at
least twice a month and medication monitoring at least twice a
month in the home but more frequently as needed (based on assessed
needs) and documents all sessions in Evolv. Attends all scheduled
psychological evaluations, psychiatric reviews and medication
management telemed appointments with youth, family and the staff
psychiatrist or nurse practitioner.Works collaboratively with
youth, family and staff psychiatrist to modify clinical
interventions, treatment plan goals/objectives, medications and
other services as needed in the event of severe behavior regression
or adverse medication side effects. Attends clinical staffing with
the staff psychiatrist or nurse practitioner at least once a
month/as needed. Provides and coordinates clinical services that
are youth focused and builds on the family and community strengths.
Works in a team context with the Wraparound Facilitator, family
members, community agencies, and stakeholders across family serving
systems. Carries out quality assurance activities as assigned such
as auditing case files. Prepares and completes program reports as
required for compliance. Attends weekly staff meetings and/or
supervisions as well as facilitate monthly case staffings with
psychiatrist. Utilizes and accepts clinical supervision to provide
quality mental health services.Is familiar with all emergency
procedures. Is responsible for sensitivity and responsiveness to
cultural differences in the organization/program service
population. Continuously work with the youth, family and Wraparound
Facilitator in the development and coordination of a
discharge/aftercare plan beginning at admission and at least 10
days prior to discharge from MYPAC. Provide around the clock
support for the child/family through on-call services. Provides
transportation when necessary. Other related duties as
assigned.Required Qualifications:Valid and current mental health
Certification (PCMHT or CMHT) and/or license (LPC, LCSW, LMSW,
LMFT, LMFTA) within 30 days of employment.Master's Degree in social
work, psychology, counseling or related behavioral health field
required. Must have at least two (2) years clinical experience
working with children and youth with serious emotional disturbances
and families with complex needs in a similar or more restrictive
placement environment.Must be at least 21 years of age, have a
valid Mississippi driver license, a good driving record, proof of
legally appropriate auto liability insurance, and must be willing
to use personal vehicle for organization business.Equal Opportunity
